How to Qualify for a Medical Marijuana Card in New York

In order to qualify for a New York marijuana card so you can legally purchase medical marijuana from licensed New York dispensaries, you must fulfill the following qualifications:

  1. Be diagnosed with a qualifying medical condition

  2. Have the official written certification from a certified New York physician

  • ​Chronic pain​ (pain that degrades health and functional capability as an alternative to opioid use or substance use disorder)

The severe debilitating or life-threatening condition must also be accompanied by one or more of the following associated or complicating conditions:

  • Cachexia or wasting syndrome

  • Severe or chronic pain

  • Severe nausea

  • Seizures or severe or persistent muscle spasms

  • PTSD

  • Opioid use disorder

How to Get Approved for a New York Medical Marijuana Card ​

If you have previously been diagnosed with one of the qualifying conditions, or believe you are currently suffering from one, you are most likely eligible for medical marijuana treatment in New York. The next step in the process is to find a certified physician to review your medical condition(s) and if appropriate, recommend you for medical marijuana treatment.

All New York medical marijuana physicians must complete a two to four hour course as determined by the Commissioner of Health. Physicians are trained in courses such as the pharmacology of marijuana,  side effects of marijuana, dosing, routes of administration and more to ensure they have all information possible to help evaluate patients.

 

Once you've identified the physician you wish to see, you will need to schedule an evaluation. During your appointment, the physician will discuss your medical condition, as well as your medical history. We recommended bringing a copy of your medical records to the appointment.

 

If your New York medical marijuana doctor is able to confirm your qualifying condition and believes medical marijuana is an appropriate treatment option for you, you will receive a signed certification from the physician recommending medical marijuana and or medical marijuana products as part of your treatment plan. After your appointment, you will need to submit the written certification to the New York Department of Health along with a $50 annual registration fee.

After your registration has been processed and approved, you will be issued a Temporary Registry I.D. Card which may be used with a government-issued photo identification until you receive your New York medical marijuana card in the mail. It takes approximately seven business days to receive your marijuana card.

 

What Information and Forms Are Required When a Patient Applies for a New York Marijuana Card?

The following are required when submitting your application:

• The application form

• The completed Physician Written Certification

• Photocopy of your New York issued driver's license or state ID, please be aware that your name and address on your application form must match what is listed on your New York driver's license or ID

• The non-refundable $50 application fee.

How to Find a Medical Marijuana Doctor in New York

In order to obtain a medical marijuana card in New York, patients must first find a physician licensed by the State of New York, schedule an evaluation, and be identified as having one or more of the qualifying conditions to receive a recommendation.
